About the CENTER

The Duke Center for Community and Population Health Improvement is a multi-disciplinary center that leverages academic, health system, and community partnerships to improve community and population health.
​
Through five key programs, the Center tackles numerous health conditions affecting persons across the life course, ​ ​while focusing on those needs viewed as most pressing by the community. ​

The Center views Durham County, NC as a model ecosystem through which best practices can be developed for improving community and population health. The Center will serve as a co-partner among many multi-sector partners to improve health through measurable interventions.
​
The Center will foster Duke’s academic health centers’ engagement into a broad community-driven initiative with the overall goal to improve the health of Durham County through partnered and measurable interventions that jointly can improve local and regional health.  We are looking for partners to help us:
​
  • Foster robust relationships and facilitate knowledge transfer among academic, community, and health system stakeholders.
  • Inform local, regional, and national health policies.
  • Identify targets for community and population health improvement at patient, family, community, and regional levels through clinical, social, geospatial, and other contextual analytic methods.
  • Rigorously study interventions to improve community and population health.
  • Develop novel analytic methods for community and population health research.
  • Train leaders to sustain and extend community and population health improvements.​
